Conflict of Interest Questionnaire

Effective January 1, 2006, Chapter 176 of the Texas Local Government Code requires that any vendor or person considering doing business with a local government entity disclose in the attached Form CIQ the vendor or person’s affiliation or business relationship that might cause a conflict of interest with a local government entity. By law, the questionnaire must be filed with the records administrator of the City of Everman not later than the 7th business day after the date the person becomes aware of the facts that require the statement to be filed. These forms should be filed with the City Secretary located at 212 N. Race St., Everman, Texas, 76140.

Conflict of Interest Statement

Effective January 1, 2006, Chapter 176.003 of the Texas Local Government Code requires certain local government officers to file this form. A “local government officer” is defined as a member of a governing body of a local government entity or a director, superintendent, administrator, president, or other person designated as the executive officer of the local government entity. This form is required to be filed with the records administrator of the local government eneity not later than 5 p.m. on the 7th business day after the date on which the officer becomes aware of the facts that require the filing of this statement. These forms should be filed with the City Secretary located at 212 N. Race St., Everman, Texas, 76140.
